– Full name: Provided only if the father was present at the time of birth registration.
– Occupation: The father’s occupation at the time of birth.
7. Informant details: In cases where neither parent was available for registration, details of the person registering the birth, including their description, address, and signature.
8. Registration details: Date of birth registration, recorded within 42 days of birth, along with the registrar’s signature.
